For the enrollment of dependent/s, please refer to 5th Month Notification enrollment guidelines and rate per dependent for your reference. Enrollment is online thru iCare’s Member Association Portal (MAP). The MAP link has been sent to your Concentrix email. Please check email from noreplymap@icare.com.ph.

You need not enroll yourself as game-changers are automatically enrolled once eligible and your HMO will automatically be activated. Please make sure to check the email from noreplymap@icare.com.ph with the MAP link. Please refer to this User Guide on how to access the MAP. You may enroll until 30 calendar days from the activation date. Reminders:

Regardless of the date of enrollment, the activation date of enrolled dependent/s shall follow the game-changer’s activation date, except for newborn child/ren / newlywed spouse dependents. Enrollment of newborn child/ren / newlywed spouse should be done within 30 calendar days from the date of celebration. Kindly fill out this iCare: Life Event Enrollment Form and ensure to upload a clear copy of your newborn child/ren''s birth certificate or marriage certificate for newlywed spouse. Provision for Common Law/Same Sex Partner: Standard hierarchy rule as that of a married principal shall apply. Both the Principal and Dependent are "Single" status, i.e. has not contracted legal marriage in the Philippines or elsewhere with anyone other than the principal or enrollee. With respect to domestic partners, there shall be no legal impediment to marry one another; as for the same gender partner, other than having the same gender, there should be no other impediment for them to marry each other in the Philippines. Living together in the same residence for at least six (6) months. For SINGLE game-changers who opt to enroll both partner and parents aged 65yrs old and below, the parents will be covered under the Voluntary-Paid Dependents (VPD) Program, with an annual premium of P24,150.00 per parent and 100% employee paid. (Please refer to the VPD Program rate and inclusions below or go to CORE). For MARRIED game-changers who opt to enroll parents aged 65yrs old and below, the parents will be covered under the Voluntary-Paid Dependents (VPD) Program, with an annual premium of P24,150.00 per parent and 100% employee paid. IMPORTANT: Full fees apply for offboarding or dependent cancellation if:

(a) the remaining coverage is three (3) months or less (e.g. April to June – pro-rated; October to December – full year);

(b) your dependent/s has/have used any benefits, including Telemed7.

ER CO-PAYMENT POLICY

A PHP 1,000 fee applies when accessing the Emergency Room of all iCare-accredited facilities. If the case is confirmed to require emergency care, the amount will be fully reimbursable except for Animal Bite cases. For more information, please go to CORE.

To file for reimbursement, kindly send the following requirements to CNX_ER@icare.com.ph. Note that the claim must be filed and received by iCare within sixty (60) days from the date of emergency room availment.

REQUIREMENTS:

Service Invoice Medical Certificate or Reimbursement Form (with diagnosis indicated and signed by the attending physician) If the availment was determined to be a genuine emergency and you submitted the complete requirements, reimbursement will be processed within 4 working days. If not valid, a denial notice will be issued within 4 working days. The determination is made by the attending physician and reviewed by iCare’s medical team. Generally, cases that are life-threatening or require immediate medical attention are classified as emergency cases.
